Community Health Worker
PAROW/GOODWOOD AREA
Position Overview:
The Community Health Worker (CHW) provides outreach, preventative, promotive, curative, rehabilitative, and palliative services. They promote health and well-being, educate communities, conduct screenings, offer basic care, counselling, and psychosocial support, facilitate referrals, support medication adherence, and mobilize communities through awareness campaigns.
Key responsibilities:
- Promote overall health and well-being within households and communities.
- Provide information, health education and promote healthy behaviour and disease preventions.
- Conduct structured household screening and profiling to identify health needs.
- Provide appropriate direct basic services including treatment for minor health problems/needs, counselling and psychosocial support for individuals or households, as defined by the CHW Scope of Work.
- Facilitate appropriate referral for health, rehabilitation and social support services as needed for individuals or households.
- Provide adherence support for people on medication and support follow-up care, including delivery of chronic medication.
- Facilitate community mobilisation and create awareness on health diseases through awareness campaigns and mobilise around community needs.
- Promote overall health and well-being within households and communities.
Requirements:
Home Base Care Certificate or equivalent.
Certification in HIV/AIDS and/or chronic illness.
At least two years of experience working with vulnerable communities.
Experience conducting health promotion and/or health education activities.
Experience providing direct basic services (e.g. treatment for minor health problems/needs, counselling and psychosocial support for individuals or households)
